Trans Abstract

Our conventional understanding of fluid transport across the brain has significantly changed over the last decade after introduction of the concept of the glymphatic system and discovery of meningeal lymphatics. The glymphatic system is not a true anatomical structure but merely a functional system for cerebrospinal fluid (CSF) and interstitial fluid exchange, whereby the CSF enters the brain through the periarterial space. This movement is driven by a few potential driver mechanisms. The CSF thereafter travels to the interstitium facilitated by aquaporin 4 channels in the astrocytic end feet and subsequently through the interstitium via diffusion and convection/advection and finally exits through the perivenous space. In this review, we describe magnetic resonance imaging (MRI) techniques that have been used or may potentially be useful to analyze the glymphatic system, together with a brief summary and discussion of limitations. MRI, a widely used clinical modality, may potentially provide deeper understanding of the pathophysiology of various diseases based on the concept of the glymphatic system.

서 론

뇌척수액의 순환은 맥락얼기(choroid plexus)에서 형성되어 거미막과립(arachnoid granulation)에서 흡수되는 단순한 과정으로 이해되어 왔다. 또한, 림프계가 없고 혈액뇌장벽(blood brain barrier, BBB)에 의해 체액의 이동에 제한이 있어, 림프계에 의해 노폐물이 제거되는 몸의 다른 부위와 달리 단순 확산(diffusion)에 의해 노폐물이 제거되는 것으로 이해되었다. 최근 글림프 시스템(glymphatic system) 개념의 등장과 뇌막 림프관(meningeal lymphatics)의 발견과 함께 이러한 이해는 크게 변화하고 있다.

글림프 시스템은 교세포(glial cell) 중 하나인 별아교세포에 분포하는 아쿠아포린 4 (Aquaporin 4)에 대한 의존성과 기능적으로 림프계와 비슷하다고 하여 명명되었다. 글림프 시스템은 단일 구조물이 아니라 뇌척수액과 간질액(interstitial fluid)의 교환에 관여하는 기능적 시스템으로, 뇌의 체액 운반 과정의 4단계 중 다음 3가지의 단계를 구성한다. 1) 투과동맥(penetrating artery)의 동맥주변공간으로 뇌척수액이 유입(influx)되며, 이는 주로 동맥의 박동에 의해 추진된다. 이러한 동맥주변공간의 용적흐름(bulk flow)은 혈관을 둘러싸고 있는 별아교세포의 종족(astrocytic end feet)에 위치한 아쿠아포린 4에 좌우된다. 2) 간질의 용질 이동(interstitial solute movement)은 확산과 이류(advection)에 의해 이루어지는데, 이류는 혈관주위공간과 백질을 따라 큰 용질의 움직임을 돕고, 보다 넓은 간질 공간에서는 확산에 의해 이루어진다. 3) 정맥주위공간과 뇌/척수 신경을 따른 간질의 용질 배출(interstitial solute drainage)이다. 그리고 마지막 4단계는 림프 흡수와 배출이다(Fig. 1) [1].

동물에서는 다양한 기법들로 글림프 시스템의 생리를 평가할 수 있지만, 사람에서는 이를 적용할 수 없는 경우가 많아 덜 침습적인 기법들이 필요하다. 자기공명영상(magnetic resonance imaging, MRI)은 임상에서 널리 활용되는 영상기법으로, 이를 이용하여 전임상 단계의 동물실험과 사람에서 다양하게 활용되고 있다[2-4]. 본 종설에서는 뇌척수액 순환(뇌척수액의 생성과 관련된 부분은 이 종설에서는 다루지 않기로 한다), 혈관주위기능 및 간질액 이동, 뇌척수액 유출 경로에 대한 영상을 중심으로 몇 가지 방법들을 소개하고 그 의미를 알아보며, 현재의 한계 그리고 향후 나아갈 방향에 대해 논하고자 한다.

본 론

뇌척수액 순환

가돌리늄 조영제의 경막 내 주입

뇌척수액의 순환을 평가하는 가장 직접적인 방법은 표지 물질을 경막 내(intrathecal)로 주입하여 변화를 관찰하는 것이다. 연구자들은 경막 내로 자기공명영상에 사용되는 가돌리늄(Gadolinium) 조영제를 주입하고 24시간 이상의 긴 시간에 걸쳐 이를 추적 관찰하였고[5], 가돌리늄 조영이 혈관을 따라서 일어나며 대뇌 피질에서부터 안쪽으로 조영 증강되는 글림프 개념에 합당한 양식의 동적 변화를 보였다. 다만, 최근 0.25 mmol의 적은 양을 주입하여도 결과를 관찰하기에 충분하다는 보고가 있었으나[6], 요추 천자라는 침습적 술기가 요구된다는 점과, 자기공명 척수조영술로 지역에 따라 이용되기는 하나[7], 경막 내로 가돌리늄 조영제를 주입하는 것은 고농도에서 독성이 나타나며[8] 허가사항 외 사용(off-label use)이라는 점, 그리고 주입 후 최종 영상 획득까지 오랜 지연 시간이 필요하다는 점 때문에 임상에서 통상적으로 사용하기에는 매우 제한적이다. 그러나 최근의 메타 분석 결과에 의하면 낮은 농도의(<1 mmol) 경우 심각한 부작용 없이 경미한 부작용만이 동반될 수 있다고 하였으며[9], 낮은 농도(0.25, 0.5 mmol)를 주입한 149명을 대상으로 한 전향적 연구에서도 일시적인 두통, 오심 등 심각하지 않은 부작용만 관찰되었으며, 이는 요오드화 조영제의 주입이나 요추 천자 자체의 합병증과 비슷한 수준이라고 하였다[10]. 향후 경막 내 가돌리늄 조영제의 주입이 임상적으로 활용되려면 장기적 안전성을 포함한 안전성에 대한 추가 연구와 자료가 필요하다.

가돌리늄 조영제의 정맥 내 주입

경막 내 주입과는 달리 가돌리늄 조영제의 정맥 내(intravenous) 주입은 일상적으로 이루어지고 있기 때문에 훨씬 접근성이 높다. 최근 사람을 대상으로 가돌리늄 조영제의 정맥 주입 후, 0.5, 1, 1.5, 2, 12시간에 뇌 실질에서 정량적 T1 값을 측정한 연구에서, 30분 경에 뇌 영역에서 급격하게 값이 변화하였다가 회복되는 양상으로 보였으며, 이는 수면 상태에 따라 차이가 있어 글림프 기능을 반영하는 것으로 생각되었다[11]. 다만, 이러한 정량적 접근도 매우 적은 양의 조영제가 뇌 실질에 유입되는 상황에서 T1 값의 변화를 보기에는 충분히 민감하지 않다는 결과와 지적도 있다[2]. 따라서 정맥 내 주입은 경막 내 주입과는 달리 뇌혈관 벽, 혈액뇌척수액장벽, 신장기능 등의 영향을 받을 수밖에 없고, 특히 초반에 측정되는 값들은 혈관 내 존재하는 가돌리늄의 양에 측정값이 영향을 받게 되는 등의 교란 인자(confounding variables)가 많아 주의가 필요하다.

뇌척수액 박동

뇌척수액과 노폐물은 확산 이외에도 뇌척수액이 글림프 시스템을 통한 대류(convection)를 하면서 노폐물이 이류를하는 것으로 제안되어 있다. 이 뇌척수액의 움직임의 추진력(driving force)으로는 심장 주기, 호흡, 신경 활동, 혈관 운동(vasomotion) 등이 제안되고 있다[12]. 이러한 추진력도 글림프 순환에 영향을 주기 때문에 평가가 필요할 수 있다.

전반적인 뇌척수액의 흐름이나 뇌혈관의 박동성(pulsatility)도 자기공명영상으로 평가가 가능하지만[13,14], 본 종설에서는 자세히 다루지 않고 글림프 시스템과 연관된 몇가지에 대해서만 소개하고자 한다. 뇌의 다양한 박동은 수면, 하루 주기리듬(circadian rhythm)을 비롯하여 여러 요소에 따라 상시 변화하고 있기 때문에 측정과 해석에 주의가 요구된다.

확산자장을 이용한 혈관주위 박동 측정

기존의 뇌척수액 평가 방법인 위상 대조 자기공명영상(phase contrast MRI)은 화적소(voxel) 내 대다수가 일관된 움직임을 보여야 하기 때문에 뇌실(ventricle)이나, 대뇌수도관 등에서만 정량이 가능하며 비교적 빠른 흐름만을 측정할 수 있다. 게다가 혈관주위공간은 크기가 일반적으로 1-2 mm 정도로 기존 방법으로는 흐름을 측정하기가 매우 어려웠다. 최근 확산 자장(diffusion gradient)을 이용하여 뇌척수액의 움직임을 측정하고자 하는 시도들이 있었다.

최근 Hirschler 등[15]은 7테슬라(Tesla, T) 장비에서 건강한 자원자를 대상으로 심장, 호흡 주기와 동기화 하여 0.45 mm의 공간해상도로 뇌척수액의 흐름을 촬영할 수 있었다. 이 결과에 따르면 기저 수조(basal cistern)를 비롯한 큰 뇌척수액 공간에서는 심장 주기가 흐름에 더 큰 영향을 주었으나, 뇌 표면 및 혈관주위공간에서는 심장 주기와 호흡이 비슷한 정도로 영향을 준다고 보고하여 이러한 추진력은 부위 특이적이라고 제안하였다. 다만, 7T 장비가 필요하고 촬영 간이 40분 가량으로 시간이 너무 오래 걸려 접근성이 떨어지고 시간 해상도(temporal resolution)가 너무 길다는 단점이 있었다. 최근 확산강조영상을 이용하여 낮은 해상도(1.8×1.8×4 mm)로 6분 이내로 촬영할 수 있는 방식도 소개되었으며 이를 역동적 확산강조영상(dynamic diffusion weighted image)이라고 명명하였다[16].

혈중 산소치 의존 신호 기반 박동 측정

자기공명뇌파(MR-encephalography, MREG)

혈중 산소치 의존(blood oxygen level dependent, BOLD) 신호는 신경세포가 활동을 하면, 주변 세동맥을 확장시키게 되고, 산소화된(oxygenated) 혈액이 유입되어 신호가 증가하는 현상으로, 호흡이나 심장주기 등이 생리적 잡음(physiologic noise)으로 영향을 미칠 수 있다. 일반적인 기능적 자기공명영상(functional MRI)의 반복시간(repetition time)은 2-3초이기 때문에 측정하는 혈중 산소치 의존 신호에 높은 주파수의 신호들이 있다면 앨리어싱(aliasing)되어 분리가 어려워진다. 따라서 자기공명뇌파(MREG) 등 매우 빠른 반복 시간(100 ms)으로 영상을 얻게 되면 심장과 호흡기의 앨리어싱을 정확하게 제거할 수 있게 된다. 이렇게 얻어진 신호를 분석해보면 심장 주기(0.8-1.2 Hz), 호흡(0.2-0.3 Hz) 그리고 매우 낮은 주파수(<0.1 Hz)로 주로 구성되어 있었으며, 심장 주기는 주로 동맥 주위에서 원심성(centrifugal)으로, 호흡은 정맥 주변으로 구심성으로 독특한 공간적 양상으로 퍼진다고 하였다. 이 중 0.1 Hz 이하의 주파수는 혈관 운동과 관련있는 파형이라는 가설을 제시하였다. 연구자들은 이 방법이 심장박동, 호흡, 그리고 혈관 운동을 분석하기 때문에 글림프 박동을 평가할 수 있는 방법이라고 주장하였다[17]. 최근 알츠하이머병에서는 정상과 비교하여 파형의 도착 지연(arrival latency)과 전파속도가 감소되어 있었으며, 해마와 두정엽 부위에서는 박동(impulse)의 전파가 역전되어 나타났다[18]. 이 방법의 제한사항으로는 짧은 반복시간으로 얻는 촬영 기법이 연구용이라 획득하고 영상을 재구성하는 데 장벽이 있으며, 촬영 시 심장박동, 호흡을 따로 측정해야 한다는 점이다.

혈중 산소치 의존-뇌척수액 연결(BOLD-CSF coupling)

Fultz 등[19]은 빠른 반복시간으로 기능적 자기공명영상을 촬영할 때 자기장의 경계부위에서 뇌척수액의 흐름을 같이 얻을 수 있는 기법을 이용하여, 수면 시 혈중 산소치 의존 신호와 뇌척수액의 박동(제4뇌실), 그리고 뇌파를 함께 측정하였다. 그 결과 비급속안구운동수면(non rapid eye movement sleep) 시기에 진폭이 각성 상태에 비해서 매우 큰 0.05 Hz 정도의 저주파의 뇌척수액 박동이 일어났으며, 이는 혈중 산소치 의존 신호와 연결되어 일어났다. 즉, 혈중 산소치 의존 신호가 내려갈 때 뇌척수액 신호는 올라갔고, 반대 상황에서 뇌척수액 신호는 떨어졌다. 이는 두개 내 공간은 한정적이기 때문에 대뇌 혈액량(cerebral blood volume)이 증가하면 보상적으로 뇌척수액은 감소해야 하기 때문으로 이해할 수 있다. 이 혈액 증가의 원인을 뇌파를 통하여 추적해 보면, 뇌척수액의 박동이 있기 6초 전, 신경세포 활동의 억제에 의한 느린 파동이 관찰되고, 이것이 기능적 충혈(functional hyperemia)을 일으켜 뇌척수액의 박동을 일으킨다는 설명이 가능하다.

이외 비슷한 접근으로 혈중 산소치 의존 신호와 뇌척수액의 박동을 함께 측정하여 이의 교차 상관 관계를 구하여 혈 중 산소치 의존-뇌척수액 연결(BOLD-CSF coupling)이라고 하였고, 알츠하이머 환자, 경도인지장애, 그리고 정상군에서 차이가 나는 것을 관찰하였으며, 이를 글림프 유입과 연관하여 설명하였다[20].

Kim 등[21]은 기능적 자기공명영상을 획득하면서 뇌척수액의 흐름을 얻는 이 방법론을 개선하여 기존 방법에 비하여 더 정확하고 정량적인 특성의 결과를 보였다.

혈관주위 기능 및 간질액 이동

늘어난 혈관주위공간(Enlarged perivascular spaces)

혈관주위공간은 글림프 시스템에서 매우 중요한 구조이며, 복합적인 원인에 의해 늘어난 혈관주위공간은 혈관 주위 기능(perivascular function), 특히 유입 부전의 표지자로 여겨지고 있다[22,23]. 최근 7T 자장에서의 연구결과에 따르면 자기공명영상에서 관찰되는 혈관주위공간은 대부분 세동맥(arteriole) 주위 공간으로 생각 되어진다. 자기공명영상에서 관찰되는 늘어난 혈관주위공간을 영상으로 평가하는 방법은 정성적 평가가 주를 이루고 있는데, 기존 몇가지의 시각적 평가(visual assessment) 방법들이 제안되어 있으며, 보통 T2 강조영상에서 정해진 부위(예: 기저핵[basal ganglia], 난형 중심[centrum semiovale])에서 개수를 구간(예: 1-10/11-20/21-40/>40) [24]별로 등급화하여 평가하는 방식이다. 하지만, 이러한 방식에는 1) 열공 뇌경색의 병변과 비교가 어려움, 2) 시간 소모적이라는 점, 3) 경험에 따라 검출 치우침(detection bias)이 있을 수 있다는 점, 4) 천장과 바닥 효과(ceiling and floor effect), 5) 추적연구(longitudinal study)에 적합하지 않다는 점, 6) 고품질의 영상이 필요하다는 점, 그리고 7) 혈관주위공간의 체적을 측정할 수 없다는 점 등 여러 제한점이 존재한다[25]. 최근에는 이러한 등급을 자동으로 평가하는 것이 가능해 지거나[26,27], 늘어난 혈관주위공간을 자동으로 분할(segmentation)하여[25], 체적을 측정하거나, 모양 등에 대한 특징을 계산하는 등의 정량적인 평가가 가능해지고 있다[28]. 이러한 방법은 천장 효과가 없고 연속형의 결과가 주어지며 채점자내, 채점자 간(intra, inter-rater) 다양성이 없어진다는 장점이 있다. 다만, 최신 영상 장치와 획득 방법에 따라서 고해상도, 고 신호 대 잡음비(signal to noise) 영상이 가능해지면서, 과거 자료에 비해 더 많은 혈관주위공간이 젊고 건강한 대상에서도 어렵지 않게 찾아볼 수 있는데[28], ‘늘어난’ 혈관주위공간을 어떤 크기를 기준으로 할지에 대해서는 아직 정확한 기준이 나와있지 않으며, 혈관주위공간을 잘 보이게 하기 위한 영상 처리로 검출되는 개수가 달라진다고 보고된 것[29] 등을 고려하면 이러한 정량적인 측정을 위해서는 여러 표준화 작업이 선행되어야 할 것으로 보인다. 또한, 늘어난 혈관주위공간은 성별, 신체비만지수, 수축기 혈압, 유전적 요인, 측정한 시간 등에 영향을 받아 개인 간 다양성이 높기 때문에, 글림프 기능의 표지자로 사용하고자 할 시에는 해석에 주의가 요구된다.

확산텐서영상(Diffusion tensor imaging, DTI)

혈관주위공간에 따른 확산텐서영상분석법(DTI-ALPS)

확산강조영상은 체내 물의 움직임을 영상화하는 기법으로, 혈관주위공간에 따른 확산텐서영상분석법(Diffusion Tensor Image Analysis aLong the Perivascular Space, DTI-ALPS)은 이름에서 유추할 수 있듯, 혈관 주위 기능을 평가하기 위하여 확산강조영상 방법 중 확산텐서영상(DTI) [30] 기반으로 제안된 방법이다. 이 방법은 확산 계수(diffusion coefficient, D)에 가장 큰 영향을 주는 백질 섬유 방향의 물의 움직임의 영향을 줄일 수 있도록, 뇌 수질 혈관(medullary vessel)과 백질 섬유가 직각으로 주행하는 부위, 즉 측 뇌실 옆의 투사 섬유(projection fiber)와 연합 섬유(association fiber)에서 관심 영역을 수동, 혹은 자동으로 지정하여 각 해당 방향으로의(Dxx, Dyy, Dzz) 확산 영상에서 얻어진 값을 바탕으로 계산하게 된다(Fig. 2). 알프스 지수(ALPS index)는 낮은 값을 가질수록 혈관주위공간 방향으로 물의 확산성(diffusivity)이 저하되어 있다고 해석할 수 있다[31]. 최근, 한 연구에서는 알프스 지수가 가돌리늄 조영제를 경막 내 주입 후 계산한 글림프 제거율(clearance)과 높은 상관관계를 보였다[32]. 알프스 지수는 다른 장비에서 영상을 획득 하더라도, 영향을 미치는 몇가지 요소들(에코 시간[echo time], 확산경사자장방향[motion probing gradient]의 개수, 머리의 자세[head position], 영상 평면[imaging plane])을 고정하면 높은 재현성을 보이며, 흔히 활용되는 일반 확산강조영상과 동일한 수준의 3개 방향의 확산경사자장을 활용하여도 기존 값들과 높은 상관성을 보이는 값을 얻을 수 있기 때문에 비교적 적용하기 쉽다는 장점이 있다[33]. 이 기법은 기존 확산텐서영상에 특별한 기법이 추가로 적용된 것이 아니라 영상 처리를 통하여 적용이 가능하기 때문에 기존 데이터가 있다면 바로 적용해 볼 수 있어 비교적 연구에 널리 활용되고 있다. 다만, 특정 부위에서 측정이 이루어져야 하기 때문에 뇌 전체나 부위별 글림프 기능을 측정할 수 없다는 점, 측정 부위의 백질의 미세 구조(microstructure)의 영향에서 자유로울 수 없다는 것을 들 수 있어 결과 해석에 주의를 요한다.

자유수 분획(Free water fraction)

최근에는 확산텐서영상의 모델 중 하나(regularized bitensor model)를 활용하여, 화적소 내 자유수 분획을 정량적으로 측정하여 간질 수분의 양의 추정값(estimate)으로 활용하고자 하는 연구가 있었다. 이들에 따르면 알츠하이머병에서 자유수 분획이 증가하는데, 이는 질병 상태에서 글림프 기능 장애에 의해 유출에 지장이 생긴 것이고, 일부 연구에서는 증가된 자유수 분획이 간질 수분의 정체라고 주장했다고 하였다. 다만, 자유수 분획의 증가는 위축, 부종, 염증, 미엘린 감소, 혈액뇌장벽 변화 등 다양한 상황에서 변화할 수 있기 때문에, 앞서 언급한 것과 같은 조직병리학적 상태를 반영하는 것인지에 대해서는 확실하지 않다. 또한, 이 연구를 제외하고는 아직 자유수 분획과 글림프 시스템을 연관지은 문헌은 없다. 따라서 이를 해석하고 사용하는 데는 주의가 요구되며, 더 검증이 필요할 것으로 보인다[34].

스펙트럼 분석(Spectral analysis)

화적소내비결집운동영상(Intravoxel incoherent motion, IVIM)

한 개의 화적소 내에서 다른 지수적인 감소(exponential decay)를 보이는 값들의 스펙트럼이 혼재되어 측정되는 경우 이들의 기여 정도를 분리해 낼 수 있다. 예를 들어 확산강조영상의 경우, 혈관 내처럼 빠른 확산을 보이는 경우 영상의 확산강조의 정도를 나타내는 b-값(b-value)을 증가시키면서 빠르게 값이 떨어지고, 느린 확산의 경우 반대의 결과를 보이게 된다. 한 연구에서는 화적소내비결집운동영상(IVIM)[35] 분석을 위해 낮은 값부터 높은 값까지, 15개의 b-값(0, 5, 7, 10, 15, 20, 30, 40, 50, 60, 100, 200, 400, 700, 1,000)의 확산강조영상을 얻은 후 비음최소제곱(non negative least squares)이라는 방법을 적용하여 한 화적소 내 분포하는 확산성을 실질, 미세혈관, 그리고 그 사이의 확산성을 가지는 중간 분획(intermediate compartment)으로 분리하여 분획의 비율을 정량적 지표로 제시하였다. 이 연구에서는 중간의 확산성을 가지는 분획의 비율이 혈관주위공간이나 보다 자유롭게 움직이는 간질액 내 수분과 관련이 있다고 해석하였으나, 이러한 수치들이 실제로 해당 변화를 나타내는 것인지에 대해서는 조심스럽게 접근해야 하며 시간에 따른 추가 연구가 필요하다[36].

T2 성분 분석

유사하게 T2가 짧은 미엘린 결합수(myelin water)의 경우는 에코시간에 따라 빠른, 그리고 T2가 긴 뇌척수액은 느린 붕괴를 보이게 되어, 여러 T2 값을 가지는 세포 외 수분이 화적소 내에 혼재되어 있을 때 여러 스펙트럼의 분획으로 분리해 낼 수 있다. 이러한 접근은 짧은 T2 (10-50 ms) 값을 가지는 미엘린 결합수 영상 등에서 많이 사용되어 왔으나, 최근한 연구에서는 보다 긴 간질액/뇌척수액(100-2,000 ms)에 초점을 맞추어 정량적 T2 값을 얻는데 널리 사용하는 CPMG (Curr, Purcell, Meiboom, Gill)법으로 25개의 에코(40, 80, 120, ..., 1,000 ms)를 획득하였고, 비음최소제곱 방법으로 여러 T2 값의 분획(60-2,000 ms)으로 나누어 분석하고자 하였다. 아직 질병이나 글림프 시스템과 관련한 연구에 적용되지는 않았으나, 간질액이나 노폐물의 분포와 관련한 통찰을 줄 수 있을 것으로 기대되고 있다[37].

혈액뇌장벽-수분교환(BBB-water exchange) 영상

기존에 혈액뇌장벽(BBB)을 평가하기 위해서는 가돌리늄 조영제를 사용해 왔으나, 이는 비교적 크기가 커서(물질에 따라 다르나, 가돌리늄-DTPA는 550 Da, 가도부트롤은 604 Da이다) 혈액뇌장벽이 상당히 손상이 이루어져야 혈관 외로 유출이 된다는 제한점이 있어 조영제에 비해 훨씬 작은 물(~18 Da)을 이용한 기법들이 개발되고 있다. 최근 동맥스핀 표지(arterial spin labeling)기법을 기반으로 비침습적으로 혈액뇌장벽에서 수분 교환(water exchange)의 정도를 영상화할 수 있는 기법들이 개발되고 있으며[38], 아쿠아포린 4 유전자 결손 쥐(knockout mouse)를 대상으로 한 연구에서 정상에 비하여 혈액뇌경계(blood brain interface)에서 교환 시간(exchange time)이 증가되었다는 것을 제시하였다[39]. 사람에게서도 유사한 기법들이 적용되고 있으며, 이러한 수분 교환의 차이를 아쿠아포린 4와 연관된 것으로 설명하기도 해, 글림프 기능을 평가 할 수 있는 수단이 될 수 있을지 주목받고 있다[40,41].

뇌척수액 유출 경로의 영상

정맥주위 공간 및 연질막하 공간(Perivenous space and subpial space)

가돌리늄 조영제는 정맥 주입 후 뇌척수액으로 그리고 혈관주위공간으로 이동하는 것이 사람에서도 알려져 있으나[42], 그 농도가 낮기 때문에 강한 T2 강조 액체감쇠역전회복 영상(heavily T2 weighted FLAIR image) 등 특별한 영상 기법이 필요하다[43]. 이러한 기법중 하나인 삼차원 위상 민감 반전회복(3D phase sensitive inversion recovery) 영상을 이용하여 조영제 주입 5-10분 후, 피질 정맥 주변으로 선형의 조영 증강이 관찰되었으며 이는 간질액으로 차 있는 연질막집(pial sheath) 혹은 연질막하 공간(subpial space)과 정맥 벽 사이 공간으로 유출되는 것으로 추정되며, 이 구조는 시상 주변부(parasagittal area)의 뇌막 림프관으로 연결될 것으로 추정되고 있다[44]. 주입 후 4시간 후에서는 이러한 피질 주변의 뇌척수액으로 조영제 유출이 관찰되기도 한다[45]. 앞으로 이러한 구조들에 대한 이해의 증가가 글림프의 유출(efflux) 기능에 대한 이해를 높여 줄 것으로 기대된다.

뇌막 림프관(Meningeal lymphatic vessel)

Absinta 등이 처음 사람 체내에서 시상 뇌막 림프관을 관찰할 수 있다는 것을 보고한 이후, 많은 연구들이 T2 액체감쇠역전회복 영상이나 삼차원 흑혈류(3D black blood) T1 강조영상 등의 방법으로 뇌막 림프관을 관찰하였다[46-51]. 일부에서는 역동적 조영증강(dynamic contrast enhancement) 방법을 사용하여 시상 정맥동 주변 혈액뇌장벽 부위의 조영제 변화의 양상을 시간에 따라 측정함으로써 흐름을 기능적으로 평가하였다[47]. 이 기법은 적용이 어렵지 않으며 위 연구에서는 6.5분 가량의 촬영으로 기능적 평가가 가능해진다는 점이 장점이다. 뇌막 림프관은 경막 내 가돌리늄 조영제를 주입 한 후 영상을 촬영하여 조영 증강 및 제거되는 양상을 관찰할 수도 있다[52]. 아울러, 이런 방법들을 사용하면서 시상 주위 뿐만 아니라 다른 큰 정맥동 주변, 뇌 기저부에 위치한 뇌막 림프관, 신경주위 등 및 연결되는 경부 림프절까지 확인이 가능하게 되어 뇌막 림프관의 구조에 대한 이해도를 높여 주고 있다[48]. 최근 조영제를 사용하지 않는 삼차원 T2 액체감 쇠역전회복 영상이 소개되었는데, 비 침습적인 6분 이내의 촬영만이 필요해 획득이 용이하다[48]. 다만, 일부에서 지적한대로 두정(vertex) 부위에 인공물이 생기는 점과[50], 고신호강도로 보이는 부분들이 뇌막 림프관에만 특이적인 것은 아니라는 점을 고려하는 것이 필요하다.

Kuo 등[53]은 혈관의 흐름을 영상화하는 데 쓰이는 유체속도 강조 자기공명혈관 조영술(time of flight MR angiography) 기법으로 고해상도의 영상을 여러 번 촬영하여 시상 정맥동(sagittal sinus) 주변에 정맥의 흐름과 반대 방향으로 움직이는 흐름을 발견하고, 이는 뇌막 림프관의 흐름이며 정맥동과 반대 방향으로 시상판(cribriform plate)을 향해 앞쪽으로 흐르고 있다고 주장하였다.

시상주위 경막(Parasagittal dura)

Ringstad와 Eide [54,55]는 경막 내로 주입한 조영제가 뇌척수액에서 시상주위 경막으로, 궁극적으로는 두개골의 골수로 이동하는 것을 관찰하였으며 뇌막 림프관과는 다른 구조로 생각하였다. 최근 해부조직학적 연구에서는 경막, 특히 시상주위 경막에 경막 통로(dural channel)라는 빈 공간이 존재하며 뇌척수액의 배출통로이자 저수지의 기능을 할 것이라고 추측된다(Fig. 3). 또한, 기존 연구들에서 뇌막 림프관이라고 언급된 구조들이 실제로는 1 mm보다 작은 림프관자체가 아니라 일부의 경우에는 시상주위 경막이 관찰된 것일 가능성을 제시하였다[56]. 시상주위 경막의 체적은 연령에 따라 증가하는 것이 알려져 있으며 최근에는 자동으로 분할하여 체적을 측정하는 방법도 소개되고 있다[48,57,58]. 최근에는 이러한 시상주위 경막이 신경면역 경계면으로의 역할이 대두되고[59], 시상주위 경막을 통하여 두개골의 골수(skull bone marrow)로의 이동이 관찰되어 뇌의 면역계와의 연관성의 관점에서 주목을 받고 있다[55,60].

자기공명영상 기법을 이용한 수면과 수면질환에서의 글림프 시스템 연구

글림프 시스템 기능은 수면 시 더 활성화되는 것으로 알려져 있는데, 이는 동물 연구에 따르면 수면시 보다 구체적으로는 비급속안구운동수면 시기에 세포 외 공간이 증가하기 때문으로 생각된다. 이러한 기전은 밝혀져 있지 않으나 노르에피네프린의 조절을 받는 이온 통로와 전달체가 관여하는 것으로 생각된다[12]. 사람에서 수면에 따른 글림프 기능에 대한 연구를 살펴보면, 가돌리늄 조영제를 경막 내 주입한 연구에서 주입 후 단 24시간의 수면 박탈 시에도 조영제의 제거가 대조군에 비해 천천히 일어났으며, 이후 충분한 수면을 취해도 대조군의 제거율을 따라잡지 못했다[61]. 다른 연구에서는 조영제를 정맥 내로 아침에 주입 후 낮 시간 활동을 한 경우와, 저녁에 주입 후 수면을 취한 경우를 비교하였는데, 수면을 취하였을 때 조영제의 제거가 더 빠르게 일어났다[11]. 하지만, 만성적으로 수면의 질이 좋지 않은 군(Pittsburgh Sleep Quality Index >5)에서는 경막 내 조영제 주입 후, 조영제의 제거가 감소되어 있지 않았고, 수면의 질이 좋은 군에 비해 조영제의 신호가 높게 나타나기도 하였다[62]. 여러 연구들이 수면과 늘어난 혈관주위공간이나 알프스 지수 등의 관계를 분석하였는데, 수면의 질과 수면 효율이 더 높은 늘어난 혈관주위공간 부담(enlarged perivascular spaces [ePVS] burden)과 연관이 있었다[27]. 최근에는 수면과 중년과 노인을 대상으로 한 연구에서, 얕은 수면은 보다 높은 늘어난 혈관주위공간 부담(ePVS burden)과 연관이 있었으며[63], 노인에서 N2 수면과 무호흡-저호흡지수(apnea-hypopnea index)가 알프스 지수와 독립적으로 연관이 있다는 보고도 있었다[64].

사람을 대상으로 한 수면질환에서의 글림프 시스템 연구는 아직 활발히 진행되지 못하고 있지만, 최근 이러한 자기공명영상을 이용한 몇 편의 연구가 출판되었다. 만성 불면증(chronic insomnia) 환자에서는 정상인에 비해서 늘어난 혈관주위공간이 더 흔히 관찰되며 그 정도는 인지기능과 연관된다는 보고가 있으며[65], 폐쇄수면무호흡(obstructive sleep apnea) 환자[66,67], 렘수면행동장애 환자[68,69], 그리고 기면병(narcolepsy) 환자[70]를 대상으로 혈관주위공간에 따른 확산텐서 영상분석법을 이용하여 글림프 시스템의 기능 저하를 보고한 연구가 있다. 향후 여러 수면질환 환자를 대상으로 자기공명영상을 이용한 글림프 시스템 연구는 더욱 활발히 진행될 것으로 예상된다.

결 론

글림프 시스템의 개념은 현재 진행형으로 빠르게 변화해 나가고 있으며 그 개념은 뇌 과학 생리학 및 의학 등 여러 영역에 영향을 미치고 있다. 자기공명영상 기법이 현재 글림프 시스템을 정확하게 영상화하기는 어렵지만, 뇌 영상에 가장 널리 사용되는 도구로서, 글림프 시스템의 여러 측면을 반영하는 영상 기법이 개발되고 활용되어 나간다면, 다양한 질환의 병태생리를 글림프 시스템을 바탕으로 이해하는 데 도움을 줄 것으로 기대된다.

Schematic diagram of the glymphatic system. The cerebrospinal fluid (CSF) is produced in mainly produced in the choroid plexus, and moves to the subarachnoid space. Then it egresses to the peripheral circulation via multiple routes such as the meningeal lymphatics, perineuronal pathway, parasagittal space and arachnoid granulations. Or it can enter the glymphatic pathway where it moves along major arteries by drivers including arterial pulsation, respiration and vasomotion. It enters the brain through perivascular space of penetrating arteries and into the extracellular space with help of the polarized Aquaporin 4 located at endfeet of astrocytes. It travels through the interstitium by diffusion or advection until it is effluxed through perivenous space and along subpial space. Leakage can occur into the subarachnoid space in this stage. Recently, it was shown that CSF may cross the dura into the skull marrow.
